Adult Family Homes
Adult family homes provide the support necessary to enable adults with disabilities to live in a home and be actively engaged in their community. The homes serve as residences for two to four persons and are licensed by the county or state. Riverfront currently provides services in eight adult family homes – three homes in La Crosse County and four homes in Rock County and one home in Jackson County. Residents receive the ongoing support they need from staff members in the home. Among the many opportunities available to residents is the chance to participate in community events, take part in recreational activities, attend religious services of their choosing, develop friendships, and play an active role in their neighborhood. Adult family homes allow individuals to become an integral part of their community in the capacity and manner they choose.
Live-By Model
The live-by program provides the support necessary to enable adults with disabilities to live in their own residences in the community. Riverfront currently provides services to four individuals through the live-by program in La Crosse County. Individuals participating in this program require fewer supports than what an adult family home provides. Participants receive support from a staff member living within the same neighborhood. In addition to regularly scheduled hours, designated staff members are standing by on-call to respond to critical needs as they arise. This allows for seamless service delivery in response to changing needs. Examples of the support provided include facilitating community integration, meal preparation, personal care, medication monitoring, grocery shopping, laundry, and money management.
